But there is a point where breaking down muscle goes too far. That is where rhabdomyolysis comes into the conversation.<\/p>\n<p>Rhabdomyolysis is a serious condition involving the breakdown of skeletal muscle tissue and the release of muscle-cell contents into the bloodstream. Those contents include creatine kinase (CK) and myoglobin, and in severe cases, the condition can contribute to acute kidney injury.<\/p>\n<p>And yes, bodybuilders can develop rhabdomyolysis.<\/p>\n<p>In fact, published case reports and athlete studies have documented it following weightlifting and bodybuilding workouts.<\/p>\n<p>In this article, we\u2019re going to break down what rhabdomyolysis is, why bodybuilders can develop it, and the signs to look for.<\/p>\n<p><em>Disclaimer: This article is for informational purposes only and should not be used to treat or diagnose any condition. You create mechanical stress on the muscle. You fatigue the muscle. You create damage that the body then has to repair.<\/p>\n<p>That\u2019s part of the process.<\/p>\n<p>Rhabdomyolysis occurs when muscle damage becomes excessive enough that muscle cells break down and release their contents into the bloodstream.<\/p>\n<p>Myoglobin is particularly concerning because it is filtered by the kidneys and, in high concentrations, can contribute to kidney damage.<\/p>\n<p>This isn\u2019t simply being sore after leg day and feeling like you\u2019re crippled.<\/p>\n<p>Rhabdomyolysis can involve severe muscle pain, weakness, swelling, dark or cola-colored urine, and dramatically elevated CK levels. However, not every person will experience every classic symptom.<\/p>\n<h2>Why Do Bodybuilders Get Rhabdomyolysis?<\/h2>\n<p>It\u2019s fair to say that no one wants to suffer from rhabdomyolysis. So, what is the reason or cause for it in the first place?<\/p>\n<h3>1.\u00a0\u00a0 Excessive Training Volume<\/h3>\n<p>One of the biggest problems is doing too much, too quickly.<\/p>\n<p>A <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/18091694\/\">published case report<\/a> described a 46-year-old man who performed four to six hours of bodybuilding exercise per day for five consecutive days. He wasn\u2019t previously active in any sports or exercise.<\/p>\n<p>What happened was that he developed muscle aches after the first day but continued training. After the final day, he developed extreme fatigue and stopped producing urine. His CK eventually reached 208,274 U\/L, and he required hemodialysis because of acute kidney failure.<\/p>\n<p>That\u2019s not hardcore. That\u2019s excessive.<\/p>\n<p>There is a difference between pushing yourself and putting your body into a situation where the consequences can become life-threatening.<\/p>\n<h3>2.\u00a0\u00a0 Unaccustomed Exercise<\/h3>\n<p>Here\u2019s another point bodybuilders need to understand. You don\u2019t have to be a beginner to develop rhabdomyolysis. You can be an experienced athlete who suddenly performs an exercise, intensity, volume, or training style that your body isn\u2019t accustomed to handling.<\/p>\n<p>One case involved an exercise physiology professor and athlete who <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/23727696\/\">developed rhabdomyolysis after returning to high-intensity resistance training following a period of detraining<\/a>.<\/p>\n<p>His workout consisted of 48 total sets of push-ups and chin-ups with no rest between sets.<\/p>\n<p>Two days later, he developed cola-colored urine and had a CK level of 59,159 U\/L.<\/p>\n<p>That\u2019s a pretty good example of why \u201cI\u2019m in shape\u201d doesn\u2019t make someone immune to rhabdo.<\/p>\n<h3>3.\u00a0\u00a0 Even Low-Intensity Bodybuilding Can Cause It<\/h3>\n<p>This is where things get interesting.<\/p>\n<p>A published case involved a 30-year-old man who <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC2628870\/\">developed rhabdomyolysis after what was described as low-intensity bodybuilding exercise<\/a>.<\/p>\n<p>He experienced weakness, painful muscle swelling, and dark urine. His CK reached 70,962 U\/L, and his creatinine was 2.1 mg\/dL. He was treated with intravenous fluids and recovered.<\/p>\n<p>So, don\u2019t make the mistake of thinking rhabdomyolysis only happens after some insane workout. It can happen after exercise that doesn\u2019t necessarily look extreme on paper.<\/p>\n<p>\u00a0<\/p>\n<p>\u00a0<\/p>\n<p style=\"text-align: center;\">\n<div class=\"youtube-embed\" data-video_id=\"CdxfVklhGKs\"><iframe loading=\"lazy\" title=\"I was Diagnosed with Rhabdo!\" width=\"696\" height=\"392\" src=\"https:\/\/www.youtube.com\/embed\/CdxfVklhGKs?feature=oembed&#038;enablejsapi=1\" frameborder=\"0\" allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture; web-share\" referrerpolicy=\"strict-origin-when-cross-origin\" allowfullscreen><\/iframe><\/div>\n<\/p>\n<p>\u00a0<\/p>\n<h2>Weightlifting Is a Documented Trigger<\/h2>\n<p>This isn\u2019t some theoretical concern.<\/p>\n<p>A systematic review examining <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/36877581\/\">exertional rhabdomyolysis in athletes<\/a> analyzed 772 patients. Weightlifting accounted for 14.8% of the cases, or 114 patients. The mean CK level at presentation was 31,481 IU\/L.<\/p>\n<p>More recent research paints a similar picture.<\/p>\n<p>A <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/41788978\/\">2026 prospective study<\/a> involving 136 patients with exertional rhabdomyolysis found that 106 cases, or 78%, followed strength training. Only five patients in that study developed acute kidney injury, showing that rhabdomyolysis does not automatically mean kidney failure. \u00a0But it absolutely can become serious.<\/p>\n<h2>Don\u2019t Confuse Rhabdomyolysis With DOMS<\/h2>\n<p>This is where bodybuilders need to pay attention. We\u2019re conditioned to believe being destroyed after a workout is a good thing.<\/p>\n<p>You can\u2019t walk.<\/p>\n<p>Your legs are shaking.<\/p>\n<p>You can barely sit on the toilet.<\/p>\n<p>You joke about it.<\/p>\n<p>Sometimes that\u2019s simply brutal delayed-onset muscle soreness.<\/p>\n<p>But rhabdomyolysis is different.<\/p>\n<p>Severe muscle pain, weakness, swelling, or dark urine after unusually strenuous exercise shouldn\u2019t simply be dismissed as \u201ca good workout.\u201d The consequences can include acute kidney injury and, in severe cases, the <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/24982707\/\">need for dialysis<\/a>.<\/p>\n<h2>The Bodybuilding Mentality Needs Some Common Sense<\/h2>\n<p>This is where I have a problem with the culture surrounding bodybuilding.<\/p>\n<p>There is always someone willing to tell you that you didn\u2019t train hard enough.<\/p>\n<p>That you didn\u2019t go heavy enough.<\/p>\n<p>That you need more volume.<\/p>\n<p>That you need to take every set to failure.<\/p>\n<p>That you need to destroy the muscle.<\/p>\n<p>That\u2019s nonsense when taken to an extreme.<\/p>\n<p>Training hard is not the problem.<\/p>\n<p>Training intelligently is the goal.<\/p>\n<p>The objective is to create a stimulus that you can recover from and continue building upon.<\/p>\n<p>Rhabdomyolysis is a reminder that there is a line between productive training and excessive muscle damage. And crossing that line isn\u2019t a badge of honor. It\u2019s a medical problem.<\/p>\n<h2>Know When Something Isn\u2019t Right<\/h2>\n<p>If you develop unusually severe muscle pain, weakness, significant swelling, or dark-colored urine after training, don\u2019t automatically assume you simply crushed your workout.<\/p>\n<p>Get evaluated.<\/p>\n<p>CK testing, kidney-function testing, and clinical evaluation can help determine what is actually happening.<\/p>\n<p>Bodybuilding is supposed to make you bigger and stronger.
